If you are an Ohio resident and are interested in becoming a client please follow the remaining steps below:

Step 2- Click on the "Continue Here" button below and submit the form for review. Once your information is received, a team member will contact you.    

Step 3- Check your email for your intake paperwork. All approved clients will be given access to the client portal to complete the required documents.

Step 4- Schedule your Diagnostic Assessment-1st Session appointment in the client portal.

*All documents MUST be completed before given access to schedule an appointment.
